Your First Appointment

When you register for your initial visit, a receptionist will request completion of a registration form that will include personal information along with a basic medical history. You will also be provided a HIPAA Privacy Policy for signature. This offers both you and Family Health Center governmental mandated protection. If available, please bring any records you might have from your previous physician. We will also request a copy of your current insurance information. On your first visit you will be assigned a permanent chart number. Please bring the card with your chart number on it to all return visits. Have it available when calling us regarding your care.

Appointments

We try to see all patients on an appointment basis and request you call in advance so we can reserve time for you. Please, do not come to the office without calling first for an appointment. If you cannot keep an appointment, please call our office as soon as possible. We make every effort to honor all time commitments and request that you extend the same courtesy to us. This courtesy makes it possible to give your appointment time to another patient. If we must postpone your appointment, we will try to notify you by telephone as quickly as possible.

Calling Our Office For Answers

Our office staff has been trained to answer most of your questions. All questions about medications, refills, patient accounts, and insurance should be made during regular business office hours. Any questions you have regarding a particular medical problem should be made through our receptionists. They will advise your physician and/or nurse that you have called. Our medical staff makes every effort to return your call on the day the message is received. If the call requires that a physician speak with you directly, the doctor will return your call at the earliest opportunity.
