Workers' Compensation

Workers' compensation patients must have authorization from their employer or insurance carrier prior to treatment. Please advise both the receptionist and the physician prior to treatment if your injury is the result of an on-the-job accident. Injured workers arriving at Family Health Center without authorization will be responsible for payment at the time of the visit. Injuries covered by workers' compensation require that we have the following information: date and time of injury, how the accident occurred, employers name, address, and the employer or insurance carrier that is responsible for payment. We also require your personal insurance information along with a copy of your card, your social security number, and your date of birth. This information is important in case your claim is denied by workers' compensation.

Release Of Your Health Information

Reports concerning your health will not be released to an insurance company or any other third party without your written authorization.

Treatment Of Minors

Family Health Center is available to render treatment to minors once authorization has been received from the responsible parent or guardian. We cannot offer services to any patient under the age of 18 without written consent for treatment. We ask that a parent accompany minor children to their appointments. Should the occasion arise when a child must be treated and an adult is unavailable, please make out receptionist aware of the circumstances.
